A key barrier that we are trying to address in our community is generational poverty caused by poor health. This issue prevents our lower income neighbors from going to work or attending school regularly. To overcome the barrier, each patient at our clinic is enrolled in our Wellness Program whereby they receive disease management care.
For example, we provide annual disease lab screening testing (Complete Blood Count or “CBC”) that provides our patients the disease testing that is critical to prevention and early detection of diseases such as diabetes, hepatitis, prostate
disease, high cholesterol, HIV/AIDS, heart disease, liver, kidney and thyroid organ disease, anemia, autoimmune disease, etc. We even have a diabetes initiative, which sees over 800 patients on a recommended 3-month cycle.
